If your pet(s) has received medical care from any other facility besides Animal Clinic of Grand Prairie, please email a copy of your pet(s)’s records to: acgpvets@gmail.com Please read the information below carefully so you are aware of any necessary requirements for you and your pet(s) to complete prior to travel. Because our veterinarian is licensed and accredited in the State of Texas, only pets whose travel is originating in the State of Texas may be issued a Health Certificate.
Resource Information for Domestic and International Health Certificate
When traveling with your pet(s), there may be animal health requirements specific to that destination. As soon as you know your travel details, contact your local veterinarian to assist with the pet travel process. Factors to consider may include meeting time frames for obtaining a health certificate, updating vaccinations, diagnostic testing, or administration of medications/treatments. Your destination state or country may have specific health requirements that must be met before your pet(s) can enter the state or country. Since export requirements are determined by each state or country and can change frequently, every time you plan pet(s) travel you will need to verify the export requirements. Some countries and states (such as Hawaii, Japan, Australia, Turkey, United Arab Emirates, etc.) require a rabies titer (FAVN) test and a waiting period after the FAVN is submitted before the pet(s) may be imported to that state or country. The FAVN test takes from 3 to 8 weeks or more for results to return, please ensure you take this time frame into account when planning your travel.
Please also note that airlines may have separate and additional requirements. Check with your airline to determine what requirements they may have, if any.

If you have additional questions or need to determine what type of form will be accepted when the pet(s) travels, please contact the State Veterinarian’s office in the destination state or the United States Department of Agriculture.

Starting August 1st, 2024, the United States is implementing more stringent requirements to bring dogs into the country.
If you are returning to the United States with your dog after traveling to a foreign country there are requirements that must be met in order for your dog to travel back to the United States. This includes dogs that are traveling to the United States from Mexico and Canada. These are independent of the requirements for your dog to travel to your destination country. The requirements you must meet are dependent on whether the country or countries you have visited in the past 6 months are considered high risk for dog rabies by the CDC. Please refer to the following link to determine if the country you are visiting is considered high risk:

High-Risk Countries: If you have traveled in a country that is considered a high-risk country for dog rabies by the CDC in the last 6 months, you will need to meet the following requirements for your dog to enter back into the United States:
Your dog is required to:
Be at least 6 months old.
Be healthy upon arrival.
Have an International Organization for Standardization (ISO)-compatible microchip.
The microchip must be implanted before the rabies vaccination is administered; rabies vaccinations that were administered before the microchip was placed are considered invalid for travel purposes into the United States.
If your microchip was placed by the breeder or the shelter, make sure the veterinarian has scanned and notated the microchip number before administering the rabies vaccine.

If the rabies vaccine is your dog’s first vaccination, or if it is the first vaccination after the microchip is placed or scanned, it must be administered at least 28 days before the

form can be completed and the dog can travel.

form endorsed by the USDA.
This needs to be completed before leaving the United States and must be completed by the USDA-accredited veterinarian that administered the rabies vaccination or a USDA-accredited veterinarian at the practice where the vaccine was administered.
If your dog’s vaccinations are not administered at ACGP make sure there is a USDA-accredited veterinarian at the practice where the vaccines are given who will complete the Certification of US-issued Rabies Vaccination for you.
There is a limited exception during the transition period for dogs that have an export certificate that has been endorsed by the USDA, our veterinarian can advise you if your dog will qualify for this exception.
Have a CDC Dog Import Form receipt.
You will need a good photo of your dog’s face and body and the microchip number to complete this form
Arrive at the location listed on the CDC Dog Import Form.
If your dog’s rabies vaccination will expire while you are outside of the United States then you will have to have the rabies vaccine updated outside of the US and meet the requirements for foreign-vaccinated dogs.
If possible, it is recommended to have the rabies vaccination updated in the US before you travel.

Low-Risk or Dog Rabies Free Countries: If you have only traveled in a country that is considered a low-risk or dog rabies free country by the CDC in the last 6 months, you will need to meet the following requirements for your dog to enter back into the United States:

Our veterinarian requires all pets pass a negative intestinal parasite exam within one month of travel date and that adult dogs pass a negative heartworm test for all Health Certificates regardless of destination.

I understand that, if I have completed a large portion of the preliminary requirements through other clinics or veterinarians, our USDA accredited veterinarian will need to spend time going through the relevant records to determine which services and requirements will still be necessary to complete through Animal Clinic of Grand Prairie. Any administrative paperwork in excess of 1 hour will require a non-refundable consultation fee of $75 in order to compensate our trained staff for their time, even in the event that the certificate is not completed through our office.

I understand that, due to the preliminary and necessary paperwork and qualification processes involved, Animal Clinic of Grand Prairie requires a non-refundable administrative booking fee towards my appointment. This will go towards the cost of my appointment, but is not refundable in the event my appointment is missed, cancelled, or rescheduled within 48 hours of the scheduled appointment time. I also understand that if any of the requirements of my health certificate are not met due to my own planning, the airline requirements, or rescheduling outside of the control of ACGP, there will be no refund or credit for services rendered.
